AIM Physiotherapy also provides physiotherapy services at Holmes Chapel Health Centre for East Cheshire NHS patients.

Patients registered with a GP practice in East Cheshire can now self-refer to physiotherapyby completing the form below.

If you have a new pain in a joint/muscle or bone that is not ‘just going away’ or a previous problem that has not been seen by a therapist within the past six months then you may benefit from seeing a physiotherapist.

If you have symptoms that have not responded to recent treatment or you are concerned by any changes in your symptoms then speak to your GP.
